Foot problems end more races than most runners expect, relative to how little training time goes into preventing them. The frustrating part is that almost none of it is bad luck - a blister is a predictable mechanical failure, and the runners who reach the finish with intact feet aren't the lucky ones. They're the ones who stopped for the hot spot instead of deciding it would probably be fine.

Why blisters actually form

Friction between skin layers, under repeated shear, with moisture and heat making the skin more vulnerable to separating. All three factors compound each other - a foot that's merely damp shrugs off friction that a genuinely wet foot cannot, and a small amount of grit inside a sock turns manageable friction into something that raises a blister within a kilometre.

  • Ill-fitting shoes - too tight, too loose, or the wrong shape for your foot - creating a consistent friction point with every stride.
  • Wet feet - stream crossings, rain, or simply sweat pooling inside a shoe over hours - which softens skin and dramatically increases friction damage for the same amount of movement.

The habits that prevent most of it

Fit shoes for swelling, not for how your foot measures at rest. Feet swell meaningfully over a long effort, especially in heat and at altitude. Leave roughly a thumb's width of space in the toe box, and for anything beyond marathon distance, size up half a size from your normal-length-run fit rather than assuming the fit that works for a ninety-minute run will hold for twelve hours.

Moisture-wicking socks, never cotton, for the same reason cotton is wrong for cold-weather layering - it holds moisture against skin instead of moving it away, and wet skin blisters far more easily than dry skin under the same friction.

Anti-friction balm on known trouble spots before you start, not after you feel something. If you already know where your feet tend to break down - between certain toes, a specific heel spot - treat it preemptively every time rather than waiting to see if this run is the exception.

Trim toenails before race week, not the night before. Long nails jam against the front of a shoe on descents and are a common, entirely avoidable source of black toenails and nail-bed blisters.

Tape known trouble spots preemptively on race day, especially for anything beyond about six hours. A strip of tape over a spot that's given you trouble before is cheap insurance; waiting to see if it acts up this time is a bet you don't need to make.

Test everything in training first. Socks, insoles, taping method, lubricant - all of it, the same rule that governs race nutrition: nothing new on race day. A product that works fine for other people's feet can be exactly wrong for yours, and race day is the worst possible place to find out.

Catch the hot spot, not the blister

This is the highest-value single habit in ultra foot care, and it costs almost nothing to practise: the moment you feel warmth or localised discomfort that wasn't there before, stop and deal with it.

A hot spot - the warning stage before an actual blister - responds very well to a few seconds of attention: dry the area if it's wet, apply tape directly over it, and keep moving. A blister that's already formed is a considerably bigger problem, costs more time to manage properly, and can genuinely end a race if it's bad enough or in the wrong location.

The math that matters here: stopping for ninety seconds now is far cheaper than the hour you can lose managing a full blister later, and immeasurably cheaper than a DNF. Runners who push through a known hot spot "because stopping costs time" have the arithmetic backward.

If a blister forms anyway

For a hot spot without a fluid pocket yet: tape directly over the area. Many experienced ultra runners carry pre-cut strips of a low-friction tape for exactly this.

For a blister that's already fluid-filled and under painful pressure: draining it is standard field practice in ultrarunning and wilderness first aid, done carefully. Clean the area as best you can, sterilise a needle (a lighter flame or an alcohol wipe both work), pierce the edge of the blister in two or three spots rather than the centre, gently press the fluid out, and leave the skin roof in place rather than removing it - it acts as a natural, sterile dressing over the raw skin underneath. Cover with tape or a blister-specific dressing afterward.

If it's not causing real problems, sometimes the better call is simply to leave it alone and keep moving rather than stopping to manage something that isn't actually slowing you down.

Between races: foot care as an ongoing habit

Check your feet during long training blocks, not just before races. Early hot spots or recurring friction points in training are exactly the same warning signs a race gives you, with far lower stakes for addressing them.

A blister that forms in the same spot repeatedly, across different shoes and conditions, is telling you something about fit or gait, not delivering bad luck at random. Worth a proper look - a podiatrist, a gait assessment, or simply a more careful shoe-fitting process - rather than continuing to tape over the same recurring problem indefinitely.

Skin toughens gradually with consistent training, which is part of why the runner doing regular long efforts on similar terrain has fewer foot problems than someone attempting the same distance cold. There's no shortcut that replaces accumulated time on feet for this specific adaptation.

I had an athlete a few seasons ago who felt a hot spot on the outside of her left foot at around the halfway point of a 50 miler, decided stopping would cost too much time with a strong result on the line, and kept going. By the finish she had a blister covering nearly the width of her foot and couldn't run normally for almost two weeks afterward - a recovery delay that cost her far more training time than the ninety seconds of taping would ever have cost her race time. The athlete I'm proudest of on this exact point felt a nearly identical hot spot at a similar point in a different race, stopped, taped it in under two minutes, and finished strong with no lasting damage. Foot care is a small, unglamorous part of race prep that punches well above its training-time cost.
